Reporting a compliment, complaint or significant event
Event report on a patient's record, Event report on an operation, the Event report tab on the New page, or Event report under Patients on the Start menu.
One page raises a record in the feedback and incidents register: a compliment (an excellence report), a complaint, an adverse event, a non-clinical event or a freedom to speak report. Opened from a patient's record or an operation, the record is about that patient, and the page says so at the top; opened from the New page or the Start menu, it is about nobody in particular.
Choose the kind, check the date and the organisation, say how it arrived (in person, by email, from a review site) and whether it is internal or external, and write what happened. A compliment can carry a rating; an adverse or non-clinical event asks what kind it was and the harm done. Initial action taken is for what was done at the time. Nothing is saved until Create is pressed, so a half-written report is never on the register.
After Create the record is named by its reference (EXC, COM, ADV and so on, then its number). Those who have Feedback/Events on their menu are taken to it, where its progress, people involved and themes are kept; everybody else sees the reference and the record waits there for them.
