Consent

The Consent fold on the patient record, the Consent and Information tiles on an appointment, and Consent library, under Patients in the menu on Start.

On the patient's record, Consent lists every consent document the patient has in one list, newest first: its date, its procedures, one tag each, and a calendar button that opens the appointment it belongs to. A line down the left is amber while a document waits for the patient's signature and green once it is on file. Press the date to open the document. New, at the top, builds a consent document for the patient from the procedure consents and the risks in the library, with every risk of every procedure printed on it and no limit on how many. Tick a procedure and a Where on the patient box opens under it: type part of a body site, such as inframammary scar, press the one you mean, and choose a side if it matters (left, right, bilateral or midline; leave it blank to take the site's usual side). A procedure can have more than one site, and each is printed beside that procedure at the top of the document, for example PbSerum to Bilateral Inframammary scars. Save puts the document on the patient's record. Labels prints their labels and wristband. The version is shown only from the second version on.

A document awaiting signature says so, and says not countersigned until you have. Under it, QR code shows a code for the patient's phone, to read and sign it there in clinic; Email sends them a link that works for thirty days; Countersign signs it yourself. Every share is written to the patient's log. Remove deletes the document for good, after you confirm, and writes a line on the log saying you removed it. It is offered only while nobody has signed the document, the patient or a practitioner, and only to the person who built it, a Reference Data holder or a Senior Manager; anybody else sees Request removal, which takes a reason and puts a task on the list of the person who built it, linked to the patient, and the row then says Removal requested. On the document's own page, QR code to sign shows the code on the page itself while the patient has not signed. A signed or countersigned document can never be removed or deleted, here or on the document's own page, and nor can consent filed before Fedelsoft built its own documents.

THE END OF A CONSENT DOCUMENT is a Statement of patient: the patient agrees to the procedure and rates how well they understood the document, from 1 to 10, then the patient and the practitioner each sign. On every printed copy the rating is a fresh row of ten numbers to circle, whatever has been rated before, with the earlier ratings and their dates printed underneath, and each person has a line to sign, a line for their printed name and a line for the date. A patient who signs on their own phone or in the portal picks their number first and cannot sign without it. After signing, the same page offers Rate your understanding again, with their own list of ratings above it, so a patient can rate again whenever their understanding has changed.

Images, under the list, is what the patient has agreed their photographs may be used for: the levels they consented to, when that consent expires, and whether the form is not sent, awaiting signature or signed.

On an appointment, Consent and Information are two tiles: what the patient agrees to and signs, and what they are given to read. Each lists this appointment's documents first, with a green edge, then those from earlier appointments, newest first, each with its date and the appointment or record it came from. Shared says how and when each of this appointment's documents was last given to the patient, or Not yet. Press a document's name to open it; an information sheet opens over the appointment to read. Remove takes an information sheet off the record it is on, this appointment's or an earlier one's, when it should not have been given; shares already made stay on the patient's log.

In Consent, New consent starts a consent document for this appointment, and an amber note says when the appointment has none yet. On this appointment's consent, Email sends the patient a link that works for thirty days, QR code to sign shows it on screen for their phone, where they read and sign it, and Countersign signs it yourself. In Information, find a sheet by its title to put it on the appointment, and Add does the same for a sheet given at an earlier visit; Email and QR code share this appointment's sheets, and Remove takes one off the visit. Adding a sheet does not send it; the patient receives it only when you share it. Every share is written to the patient's log and listed under its own table.
