Readiness for surgery

On the operation note, on the operating list and on the booking form.

For a surgeon who has the check switched on, Fedelsoft judges every night, and whenever a record the check reads changes, whether each of their operations still to come is ready. Eight things are checked: two consultations with the surgeon, at least fourteen days from the first consultation to the operation, preoperative photographs, the consent forms the patient has read and acknowledged in their portal, an accepted quote with an invoice raised from it on the operation, the information guide shared, the nicotine record on the chart with the patient's acknowledgement of the nicotine policy, and no flagged medicine unless the patient has been asked to stop it and agreed. The number of consultations, the days, and how long before the operation each item falls due are the surgeon's settings, not fixed.

Readiness for surgery, on the operation note, shows each item with what was found, when it falls due and what puts it right. An item that is missing but not yet due says Not yet due and tells nobody. The operating list and the booking form show Ready for surgery, or Not ready with the first items missing. The waiting list switch and the Mark waiting list green button are no longer used for a patient the check covers.

Where something is missing and falls due, the people who can put it right are told once. The admin items (consultations, the fourteen days, photographs, the quote and its invoice, a missing pathway, and a second record of the same operation) are one shared task for the organisation's reception pair, titled Surgical readiness with the patient and the date: either of them can complete it and it closes itself when everything is in order. Fewer than fourteen days from the first consultation is a booking error for the team to put right, and the patient is told nothing. The surgeon's own items (the information guide, a flagged medicine, a chart problem with nicotine) are one task for the operating surgeon. The patient hears only about their own two items, in one email when they fall due, and never more than once.

A week before the operation, using the payment chaser's own working-day rules, whatever is still missing is added to the decision task the chaser raises that day, or, where there is none, is marked on the checklist task as A week to go.

A flagged medicine on the patient's record shows a box, Asked to stop, agreed, on its line on the checklist: ticking it records who and when, and a ticked medicine no longer holds the patient back. For an oestrogen only, the route decides: a patient taking it by patch, gel, spray or the like is not held back, and one taking it by mouth, or with no route recorded, is. A combined oestrogen and progestogen medicine has a route for each, chosen on its line on the checklist. Every other flagged medicine holds the patient back whatever its route. The operating surgeon can mark any item Reviewed, with a reason, which is kept and can be withdrawn; it counts as done while it stands.

When the check finds an operation ready, Fedelsoft raises Confirm patient for surgery for the theatre administrators where the operation's status is provisional, on hold or similar, and Prepare booking form for the operating surgeon, as the waiting list switch did.

The Nicotine tab on the episode records a patient's status, which nicotine products they use (cigarettes, e-cigarettes, roll-ups, cigars, pipes, tobacco, nicotine pouches, gum, patches, snuff or another) and whether they agree to stop. The health questionnaire asks the same of a patient who says they use nicotine, and fills the tab from the answers.

Settings, Bookings and theatre, Readiness for surgery, sets each surgeon's rules: whether the check is on for them, how many consultations, the cooling-off in days, and for each item how many days before the operation it falls due. Choose a surgeon and Add to set one up; it starts switched off. Whether the check acts at all, raising tasks and sending messages, is a separate switch that is not on that screen.
